The RF Health Care plan provides hospital, medical and prescription drug benefits for regular and postdoctoral employees and their eligible dependents. The plan features two Anthem Blue Cross Preferred Provider Organization (PPO) Plans as well as a selection of Health Maintenance Organizations (HMOs) that are available based on your location. Postdoctoral classified employees are also eligible to participate in the Graduate Student and Postdoctoral PPO Plan through Anthem Blue Cross. If you enroll in the PPOs, Anthem Blue Cross will provide you with a benefits handbook, and your prescription drug benefits will be provided by Express Scripts. If you enroll in an HMO, you will receive an HMO handbook and a certificate of coverage.

The Health Insurance Portability and Accountability Act (HIPAA) protects the privacy of your "protected health information" (PHI). Please refer to "Your Privacy Rights Under HIPAA" in the Benefits Handbook or Postdoctoral Benefits Handbook.

The member handbook and certificate of coverage for the health plan you choose, along with your Benefits Handbook or Postdoctoral Benefits Handbook, constitute the ERISA Summary Plan Description (SPD).

The RF also provides to all health plan participants a medical decision support service called My Medical Ally. This service provides the latest information and support for any health condition so that you can be better informed when you talk with your doctor. Read more (login required).
