Vision Insurance - Enroll

You may enroll via Employee Self Service (www.rfsuny.org/selfservice) during your initial 60 days of eligible employment, within 60 days of marriage or birth/adoption of a child, and annually during Open Enrollment. Enrollment at any other time must be done by submitting a completed Benefits Enrollment form to your campus Benefits Office. 

Late enrollment

If you delay enrollment in a Vision Plan beyond 60 days from your date of eligibility and you have met the waiting period, your coverage begins on the day your campus Benefits Office receives your completed enrollment.

If you delay enrollment in a Vision Plan beyond 60 days from your date of eligibility and you have not met the waiting period, your coverage begins once your campus Benefits Office receives your completed enrollment, and the waiting period is met based on the date of eligibility.

You may not change vision plans except during open enrollment and/or a qualifying event.
