In the event of the death of one of your dependents, please contact the campus Benefits Office. They will ensure you have updated all of the necessary benefits. Below are some of the things you should consider:
If you need to remove a dependent and/or change health coverage as the result of a death, please complete the Health Plan Enrollment Form.
A change in family status may lead to a change in federal and/or state withholding exemptions. Contact your Payroll Department to change your withholding exemptions.
