Vision Insurance - Using Vision Benefit

Step 1 – Find a Provider:

To use the vision care plan, members must first find a network provider. To find a network provider, you should:

call Davis Vision at 1-800-999-5431 to access the Interactive Voice Response (IVR) Unit, which will provide the names and addresses of the closest network providers.

OR

access the Davis Vision website at http://www.davisvision.com and use the "Find a Doctor" feature.

Step 2 – Schedule an Appointment:

Once you have chosen a network provider, you should call to schedule an appointment.

When scheduling, make sure to identify yourself as a member or covered dependent of the Research Foundation of SUNY Graduate Student Employee Health Plan using the Davis Vision Plan and provide your member ID number.
