Health Insurance - Enroll

Coverage becomes effective on the date of hire, eligibility, or fellowship appointment, if the graduate student or fellow enrolls within 45 days. If the student or fellow does not enroll within 45 days of becoming eligible, there will be a 30-day waiting period before coverage begins.

Employees or fellows who meet the eligibility criteria can also enroll in the plan during the open enrollment period from November 1 to November 30 of each year.

To enroll, complete the Research Foundation (RF) Graduate Student Employee Health Plan Enrollment Form and return it to your campus Benefits Office. 

To Enroll a Domestic Partner:

The following forms must be completed by the employee or the fellow and submitted to your campus Benefits Office.

• Benefit Plan Affidavit of Domestic Partnership
• Affidavit of Financial Interdependency
• Graduate Student Employee Health Plan Enrollment Form
• Dependent Tax Affidavit for Enrolling Domestic Partner in the Benefits Program, if the employee's domestic partner is a dependent according to IRS guidelines (not applicable for fellows)
• Proof of One Year Residency

Employees and fellows must produce two documents that substantiate financial interdependency with their partner. Examples of acceptable documentation are provided on the Affidavit of Financial Interdependency.

To enroll children of a domestic partner, complete a Statement of Dependence of Domestic Partner's Child for Participation in the RF Health Insurance Plan.

To Enroll a Spouse (same or opposite-sex):

• Complete the Graduate Student Employee Health Plan Enrollment Form
• Submit the form(s) with a copy of your marriage license or certificate
